In addition to the Application, you can expect us to request the following materials: 
  • Current P/L statement
  • Last 2 years financial statement ( balance sheet & income statement )
  • Articles of incorporation and any amendments
  • Income tax returns ( last two years ) Top 2 pages only
  • Payroll Tax returns, form 941( last 2 quarters )
  • Proof of payroll tax deposits.
  • Term Sheet for proposed Debt or Capital Funding

      

    Depending on the financial structure and necessary security, we may also request:

    • Current Aging Schedule for Accounts Receivable
    • Details on other assets

    Once we receive your application, we will advise specifically which of the above documents we will need.
